You forgot CARMEN MIRANDA, the Brazilian Bombshell star of many MGM, 20th Century Fox musicals. Appearing as a guest on a weekly tv variety show, she finished her song to applause, curtains closed as she took bows, and stayed closed, leaving 1 of her legs exposed to the audience. Stage hands must have pulled her body behind the curtain as her leg disappeared from sight. It was GHASTLY!
Plus Tommy Cooper suffered a similar fate, dying on stage 15th April 1984 from a heart attack 🙁
Comedian Dick Shawn died while performing his act at the University of California, San Diego. Age 67. As with Cognito, the audience thought his collapse was part of his act.
I believe you can add Tiny Tim to this list, who suffered a heart attack on stage and died on the way to a table

Tommy Cooper another great comedian passed on stage. One of the things he used to say was “Just like that” I can still hear his voice now saying that
